A Streamlined Process
Designed for the Medicare’s patient’s initial Welcome to Medicare or Annual Wellness Visit, the visit includes the health risk assessment and provider consultation. Following all CMS guidelines, written 5-10 year Personalized Prevention Plan of Services (PPPS) are generated for each and every patient.
Based on the results of the baseline health risk assessment patients are segmented into high, medium and low risk categories and are verified for Chronic and Pre-Conditions. 100% of patients are flagged for all eligible services identified through the patient’s unique PPPS such as:
- Screenings
- Exams
- Tests
- Shots
- Counseling
- Training
- Therapy
70% of patients routinely are verified as presenting with 2+ chronic conditions and are thus eligible for chronic care management. These patients are enrolled in our Chronic Care Management program where they will receive personal non face-to-face care from an Oculus team member on a monthly basis.
